Flu Vaccine
This year, the school age immunisations service has introduced an online consent form. You’ll be able to use this to give permission for your child to receive the nasal flu vaccination in school. The form can be accessed at www.leicsandrutlandimms.co.uk. When information is sent out from school about the nasal flu programme, it will include a unique school code which you will need to input, alongside your contact email address.
The online form will make the process easy and efficient, and will avoid letters being lost in transit. The form meets all of the NHS security standards for patient information.
Please be aware when you complete the form, that you need to input the school code without any spaces or additional punctuation. If you’re having problems submitting the form, it’s also a good idea to double check that when you confirm your email address, it matches the one originally submitted.
